First Day Forms Packet

Welcome back Mavericks! In this email you will find our Digital Start of School Packet. These forms need to be filled out and returned each school year. Forms this year are fillable. Families will need to click to open each link, fill out the form, save the form for their records and email each document to Ms. Dolan at jadolan@seattleschools.org. If you would prefer paper copies of the forms please email Ms. Dolan. Forms are available in multiple languages, please let Ms. Dolan know if you need the forms printed in another language. Paper copies of the Start of School Packet will be sent home via student mail if requested by the guardian.

All families will need to digitally fill out and email to Ms. Dolan:

• Emergency Information and Student Release
• FERPA
• Health Information Form

Only fill out these forms if they pertain to you and email to Ms. Dolan:

• 504-2 Survey to Identify Students with Disabilities
• Indian Education Program, Native Education
• Student Meals, Free and Reduced Lunch Application
• Student Housing Questionnaire

Fees are CONTACTLESS! Please do not send cash or checks. Use the links below to pay with either a credit or debit card.

• School Supply Fee $25
• ASB Fee $10
• FY22-23 Student Device Program Fee $25 – if you have a SchoolPay account you can pay the fee via SchoolPay. Link to pay the fee is not currently available.
