This has been a great year at McClure! It is hard to believe the 24-25 school year is nearing the end. It has been a pleasure working with many of you and your children over the year. For some, I will see you back in the fall, for others I wish you well on your journey. The following has information is regarding planning for next year at any SPS school.
Please note that even if your child had medication or a Care Plan at school this year, we must have updated documents for each new school year. Please review the following checklist in order to have everything ready to begin the school year in the fall.
If your child requires medication, and/or nursing treatment during the school day
____For all daily or over the counter medications as needed please complete a Medication Authorization Form
Per Washington State Legislature (RCW 28A.210.260)
Without this authorization form completed, staff cannot administer or allow students to tke any medication. This includes over the counter and homeopathic supplements
One form is required for EACH medication
____For life-threatening conditions please use appropriate form for each emergency medications:
- Epi Pens should come with severe life-threatening allergy form and Assessment form
- Inhalers should come with asthma form and Assessment formSeizure and diabetic medications should come with a school care plan from your doctor and a diabetic or seizure assessment form
- All prescription and over the counter medications must come with their own form.
- These forms must be signed by a parent/guardian AND your health care provider.
*Students ARE allowed to self-administer their medication in certain situations, but it must be noted on the form.
_____ Pick up any medications currently at school, even if your child will need them again next school year. We cannot take responsibility for any medications left in the health room over the summer break. Please let me know your plan to retrieve medications if you will be leaving before the end of the year. Lookout for a Talking Points message in the coming weeks and respond how you would like your student’s medication returned!!
_____ Updated Individual Health Care Plan
- The nurse will send you the care plan from last year via individual email by the end of the summer.
- Please respond quickly with any changes or updates.
- If there is a major change in a life-threatening condition, please include a note from a health care provider supporting the change.
_____ Before the first day of school: Return the authorization form, care plan, and bring the medication to the School Nurse. Check the expiration date of your child’s medication. If possible, obtain a medication that will not expire during the 2025-2026 school year.
_____ Students with daily medications taken at home: Please provide the school nurse with a 3-day emergency supply of medication in case of a disaster for which students are sheltered at school. These must also be accompanied by a completed Authorization for Medication at School form.
Additional notes:
- All medications, especially emergency medications, must be in their original container with a label, current and unexpired.
- If your child has any medical needs or needs to be excused from certain activities, please provide a letter that specifies these needs and limitations. Excuses from physical activity for more than a week, require a letter from your health care provider.

- If you are moving on to a new school/high school, please take form or fax it to the appropriate school nurse. Remember emergency medications must be in the building on or before the first day of school. I wish you and your student all the best!

- Vaccinations: If your child is missing vaccinations or has an incomplete record, please request a current medically verified immunization record that has documentation of the missing immunizations from your health care provider. ALL 7th Graders require an up-to-date TDAP Booster. Please schedule an appointment with your health care provider for the vaccinations that are missing.
- If a medically verifiable record is not available, please check with your provider about options to obtain a titer to check for immunization status or re-vaccination options.
